If you have received ketamine treatments at another center and are continuing your therapy with us, you will need to provide us with your most recent treatment records. Once we have reviewed these records, the process is as follows:

  • Complete Required Documentation: Even if you have provided similar information at your previous center, you will need to fill out our Medical and Psychiatric History Form, Consent Form, and Fee Sheet.
  • Schedule Your Next Treatment: Based on the information provided by your referring psychiatric health practitioner, you will be scheduled for the next treatment in your current series, rather than starting a new series. This ensures that your ongoing care is consistent with the plan established by your referring practitioner.
